If you have enrolled online using a Company Name, please complete the appropriate form (Forms 1-5 attached) and submit that form and this coversheet to PrimeMyBody via Fax at 972.692.5366. Please include your Affiliate ID and username where indicated on the form.

An Affiliate seeking to change sponsors must submit a properly completed and fully executed Sponsor Transfer Form, which includes the written approval of their sponsor. Affiliates need to allow up to seven (7) days for the change in sponsorship. For any request made three (3) days after enrollment, fees will apply.

Looking to make a placement change within your organization? Fill out the Placement Transfer Form, and follow the directions to ensure your placement change is authorized. If a request for change in placement is made within three (3) days of enrollment and does not involve more than three (3) Affiliates or moves, than PrimeMyBody will waive the processing fee.

An Affiliate's business may be sold, transferred or assigned to another individual or entity only afterPrimeMyBody corporate has approved the request in writing, which may be withheld at its sole discretion. The sale, transfer or assignment of a PrimeMyBody business may be subject to certain limitations, terms or conditions.
